Responsibilities
The Care Navigator (LMSW) functions as part of a multidisciplinary health care team working with providers and other clinical staff at Kelsey-Seybold Clinic. The social worker acts as a resource to physicians and staff, as well as a liaison for the patient.
The LMSW assesses social determinants of health and helps navigate patient and caregivers to appropriate resources/programs, removing barriers to care and wellness. The LMSW teaches and empowers the patient to make proper use of the health care system, while improving the outcome of care. He/she will collaborate with other health professionals, facilities, community programs, and vendors to ensure resources are set-up in a timely manner.
The LMSW Navigator may perform home visits, e-visits, video visits, and may work at multiple clinic locations.

Salary Range: $67,484.90 – $87,333.40 (Pay is based on several factors including but not limited to education, work experience, certifications, etc.)
Qualifications
Education
Required: Master’s degree in social work from an accredited university.
Preferred: N/A
Experience
Required: Minimum of 3 years of experience in medical social work
Preferred: Experience with EPIC
Case Management or Home health experience
License(s)
Required: Current license by the Texas Board of Social Work Examiners as a LMSW
A current and valid Texas driver’s license and proof of insurance
Preferred: N/A
Special Skills
Required: Proven communication and problem-solving skills.
Basic computer literacy

Required: Experience working with a wide variety of patients from all ages, socioeconomic and educational levels.
